About the Adult Future Options Service

The Adult Future Options Service supports young people with a disability transitioning from children’s services and adults who need long-term support including those with learning disabilities, physical disabilities, sensory impairments and continuing healthcare needs.

The service works closely with families, residential settings, commissioned providers, health, and voluntary partners to provide joined-up support and deliver:

  • Assessment, Care and Support Planning, arranging support and reviews of people living in community and residential settings;
  • Delivery of Sheffield City Council CQC regulated support (Respite, Day Activities, Supported Living).

There are 4 main services, all of which are represented in the main contents list of this procedure's manual:

Continuing Healthcare Team (CHC): Working with people whose care and support is part funded by health.

Care and Assessment:

  • Adults Future Options Community Teams that support people living in their communities;
  • Direct Payments Review Team - working with people who receive direct payments for their care.

Adult Future Options:

  • Adults Future Options Supported Living Teams that support people living in supported living accommodation across the city;
  • Adults Future Options Residential Teams that support people living in residential settings across the city and out of city;
  • Provider services - supporting people in Sheffield City Council day support, shared lives, supported living and short breaks services;
  • Sensory Impairment Team - supporting people with sensory impairments.

Provider Services:

Transitions team - support for young people who are transitioning from children’s services to adult services.
