About the Living and Ageing Well (Long Term Support) Service

The Living and Ageing Well (Long Term Support) Service supports older adults, people living with dementia and people living with long-term medical health conditions.

The service works closely with families, residential settings, commissioned providers, health, and voluntary partners to provide joined-up support and deliver assessment, Care and Support Planning, Arranging Support and Reviews of people living in community and residential settings.

There are 4 main services, all of which are represented in the main contents list of this procedure's manual:

Hospital Social Work & Out of Hours Service: Responsibility for assessing need and arranging support to facilitate discharge from hospital. Fulfil duties under making safeguarding personal and support complex situations within the hospital setting. Hospital specialist social workers who provide support for people with specific medical conditions. Delivery of the city out of hours adults care and well-being social work service.

Residential Care & Assessment Services: Responsibility for care, assessment and review within short term and long-term residential bed settings. Oversight of safeguarding, mental capacity and deprivation of liberty concerns in residential settings.

Community South & North Care & Assessment Services: Responsibility for care, assessment and review within communities across Sheffield. In addition, development of a community partnership with primary care and voluntary sector.

Response and Enablement: This includes city wide alarms, technology enabled care, response and enablement, joint falls and pharmacy support.
