About the Living and Ageing Well (Short Term Support) Service

The Living and Ageing Well (Short Term Support) Service supports young people, working age adults and older adults.

The service works closely with families, residential settings, commissioned providers, health, and voluntary partners to provide joined-up support and deliver:

  • Assessment, Care and Support Planning, and Review of people living in community, including those returning home from hospital;
  • Occupational Therapy services, equipment and adaptations;
  • CQC regulated homecare and enablement support both commissioned and provided by Council.

There are 3 main services, all of which are represented in the main contents list of this procedure's manual:

Occupational Therapy and Adapted Housing: Provides occupational therapy services to both children and adults and are a key service in prevention and enablement. This service also manages the Disabled Facilities Grant, the Integrated community equipment loans service and medical priority for rehousing.

Short Term Intervention Service: The Short-Term Intervention Team (STIT) is our in-house reablement provider service who supports people to return home after a period in hospital to regain their independence in the home.

City Wide Care Alarms and Enablement Team: Response to customers who subscribe to the care alarm service in their homes. The team also supports individuals often in crisis for a short period of time to stabilise and enable them back to be independent.
